For Class A Diseases: During business hours, please call  740-342-5179, option 2.

After business hours, please call
740-684-0004  to reach the on-call nurse.

 

For all other reportable diseases: Fax lab report and/or completed  Ohio Confidential Disease Report Form  to the health department at  740-342-5540 or call the health department at
740-342-5179, option 2 to report the communicable disease.
